Tools to Support Timely Post-Discharge Mental Health Follow-Up Care

As a reminder, in 2025, NCQA updated two HEDIS® caremeasures for post-discharge mental health appointments. Follow-up care is not limited to a licensed mental health provider. Both medical providers and non-licensed mental health providers can close gaps by conducting the appointments.

Effective Jan. 1, 2026, changes to Peoples Health plans include benefits and cost-sharing amounts. We continue to serve all Louisiana parishes and administer 17 Medicare Advantage health plans.
