New Plans and Plan Types*
We offer five new plans for 2025:
- One additional D-SNP for people with both Medicare and full Medicaid and our most robust plan of this type
- Three C-SNPs designed for people with diabetes, chronic heart failure or cardiovascular disorders
- These plans offer lower costs for specialist visits and a flexible over-the-counter items benefit with a healthy food component
- Like all of our plans, with the exception of our D-SNPs, these also provide a UnitedHealth Passport® benefit for access to plan-covered services when plan members travel within the United States but outside the plan service area
- An HMO-POS plan without Part D coverage for those who don’t need it or who have it through another source
Note: Members enrolled in a C-SNP must have their chronic condition verified by a provider within 60 days of the plan effective date. A plan representative will reach out to the diagnosing provider as soon as possible after enrollment to verify the condition. Providers will be given information about how to complete a chronic condition verification form or call in the verification.
Plan Name Change
The plan name for Peoples Health Medicare Advantage LA-0004 is changing to Peoples Health Medicare Advantage Giveback LA-4. This plan continues to offer a generous Part B premium reduction.
Part D Changes
Medicare is eliminating the Part D coverage gap stage (the “donut hole”) for 2025. This means there will be only three drug payment stages: the yearly deductible stage, the initial coverage stage and the catastrophic coverage stage. Additionally, the catastrophic coverage stage threshold is being lowered to $2,000, which is great news for Medicare beneficiaries, including your Peoples Health patients. Once the threshold is met, patients pay $0 for Part D-covered drugs for the rest of the year (note that your Peoples Health D-SNP patients pay $0 for all covered drugs in all drug stages).
