The Centers for Medicare & Medicaid Services requires plans to offer annual training to contracted providers about special needs plans (SNPs). We encourage you to complete and attest to our SNP Model of Care Provider Training prior to engaging with our SNP members. Your participation will help meet quality performance requirements and ensure the best health outcomes for these patients. The training defines the model of care, the population of our members in these plans and our care coordination efforts to address their needs.
