Preventive health checklists to identify gaps

In our ongoing commitment to improving health outcomes, we’re sending all our members a checklist of generally recommended care to help them take actionable steps to address preventive care needs in early 2026.

We encourage members to share the checklist with their primary care provider to determine the services right for them.

Consider this before year-end: See patients overdue for a blood pressure screening or an A1c test.

Get a report of your Peoples Health patients with open quality gaps.

Log in to the UnitedHealthcare Provider Portal with your One Healthcare ID and access Peoples Health Member Viewer (how you access this tool has changed). Choose the Additional Tools menu, then look in the Additional Tools section. Once in Member Viewer, select Provider Portal Quality Measure from the Criteria drop-down menu. To view your full panel, choose List All.

Health fair to engage members and close gaps

To further close gaps, we actively engaged our members through a member-only and invitation-only health fair on Nov. 14. The goal was to provide accessible opportunities for free preventive care and education, helping members take meaningful steps toward better health.

Services offered included A1c testing, blood pressure screenings, bone density scans and retinal eye exams, along with scheduling for mammograms, colon cancer screenings and kidney health evaluations. The fair also featured a cooking demo and a chance for social interaction, along with flu vaccination and representatives who answered plan and medication questions.

Ways you can help patients adhere to medication regimens:

  • Prescribe 100-day or 90-day supplies
  • Encourage mail-order programs
