The goal of our Physician Initiatives Program is to assist primary care physicians with the early screening, diagnosis and documentation of health conditions that are prevalent and often under-identified in Louisiana residents.
The following initiatives can have a direct impact on the health outcomes of your Peoples Health patients, which is why we are excited to work with you and your team on the initiatives:
- Qualifying as a 4-star or higher provider for meeting certain quality measures
- Completing common lab panels to monitor health and reduce risk for certain conditions
- Reducing hospital readmissions through post-discharge medication reconciliation during a follow-up visit or phone call within seven days of a patient’s discharge
