We encourage you to compete and attest to our annual Special Needs Plan Model of Care Provider Training. Your participation in Peoples Health care-planning activities for your Peoples Health patients in a special needs plan will help meet quality performance requirements and ensure the best health outcomes for these patients. The training defines the model of care, the population of our members in these plans and our care coordination efforts to address their needs.
If you are also a contracted provider with UnitedHealthcare, the attestation for completing this training counts for both UnitedHealthcare and Peoples Health.
